Care Model
We provide integrated medical and behavioral treatment, in-market care coordination and SDOH gap closure. Our team includes physicians, advanced practice nurses, community-based peer specialists and social workers.

Population Served
All members with any SUD diagnosis in claims in a defined geography. Typically, we focus on an MSA plus surrounding counties.

Economic Model
We prefer to take full medical and behavioral cost capitation risk on the SUD population. This structure aligns our incentives. We only make a profit if we deliver cost savings back to you.
