You can stay with your current doctor or work with us to find a new one. Your dedicated Wayspring care team comprises of clinicians, nurses, behavioral health advocates, and social workers. No matter if you stay with your current doctor or work with our team, we will listen and be there to help you get the care you need.

You can count on your dedicated care team for connection to foundational health services, transition services, peer recovery support, access to a recovery referral community, assistance with appointments, and other community-based resources and supports for employment, housing, finances, transportation, food, and legal support. Once you determine you’d like to make an appointment, we’ll set up a time on the phone to meet at a clinic or at a location of your choice. Our team will help create a care plan for your unique health needs. You need to bring your ID and insurance card. It’s also helpful to bring a list of medications that you would like to review with your provider.

Your care plan can include how to better manage chronic conditions like diabetes, asthma, or high blood pressure. Your care plan even includes behavioral health – we can help you find a therapist or psychiatrist right for you. Lastly, your care plan can connect you with community resources for housing, employment, or treatment of substance use disorder.

After creating your care plan, you and your team will work together to reach your goals.
